There are plenty of reasons why it can take a long time to settle a case. Just based on the facts you presented, including your 3 months out of work and 6 months in physical therapy, I can certainly envision an attorney waiting to settle in order to determine the true extent of your injuries and prognosis for future treatment.
You and the insurance company have to agree to settle your case. If the insurance company doesn't agree, your attorney cannot make them settle. Your option is to file a lawsuit, litigate and go to trial, if necessary.

You don't say who "they" are. Your attorneys? Make sure they protect the statute of limitations by filing a lawsuit within 2 years. Sometimes matters go to trial and don't settle. Your insurance should have help with your bills.
You have a two year statute of limitations, so either your attorney settles this within that time frame or files suit to protect your rights. I imagine your attorney believes they can settle this one so they are trying to do so. Just stay in contact with your attorney and be sure they either settle or file suit before those two years run.
